According to a CHP report at approximately 8 AM on January 13, 2015 an unknown driver was traveling westbound on Hwy 4 in a white Chrysler PT Cruiser, west of Horseshoe Drive and was passing traffic in heavy fog.
According to a witness and the involved party the fog visibility was approximately 25-30 feet.
David W. Tubulino (60) of Valley Springs was driving his Ford F-150 eastbound on Hwy 4 west of Horseshoe Drive at approximately 45-50 MPH and towing a trailer.
Tubulino observed the white Chrysler traveling directly in his lane as it was passing other vehicles.
Tubulino turned right (south) to avoid the head-on collision. He then immediately turned left, attempting to come back onto the eastbound lane.
However, instead Tubulino lost complete control of his Ford and went across all lanes and collided with a steel guard rail on the north shoulder.
There was no contact between the two vehicles and the witness was able to substantiate Tubulino's statement.
After the collision, the driver of the Chrysler left the scene on westbound Hwy 4.
